directed but failed to meet your anticipated goal, we will refund to
you the entire manufacturer's suggested retail price. Please follow
the directions below and allow thirty days to process your refund.
Remove the entire easy peel off label from the bottle and mail it to
us prior to the expiration date which is individually stamped on the
lower left front of the label.
Enclose the original sales receipt from your retailer. The receipt
must have the name of the the retailer, the store location, and the
date of purchase.
Enclose a photocopy of proof of identity such as your driver's
license or other picture i.d.
Enclose a copy of the failed toxin analysis. This is typically a one
or two page laboratory report which indicates the type of analysis
performed and the specific measurements of each procedure.
Your request for refund must be postmarked within 30 days of the
date of your toxin analysis.
Include your telephone number and area code. Our primary focus
